Open Mic Night – Performers Form

Thank you for your interest and support in our Open Mic night in aid of Dreams & Wishes Charity.  Please complete this form as fully as possible. This will help us to ensure we have everything required to ensure the evening goes smoothly (honest it will)!  Due to limited time and the no. of performers who have expressed an interest, this event is ‘first come first served’ so fill out this form asap.  We’ll confirm your space as soon as possible.
